 In order to better serve you, please review and acknowledge the following:  the credit card authorization form, that you understand the vendor’s cancellation policy, and whether or not you would like to insure your vacation. This form does not contain any credit card information and will only be valid once your agent has your updated credit card information on file. It will be sent directly to your agent.

If your payment is a recurring payment, please indicate the amount of your payment and when you would like it paid. You may also simply say “by my request” and notify your agent when you would like to have a payment made.
